Despite one of the highest levels of government health expenditure per capita, Iran has surprisingly poor health care. Furthermore, despite having 42 state medical schools, Iran is only ranked 138th in the world for doctors per capita. There is a significant geographical divide between the urban and rural areas of Iran and 27% of Iranians have no access to health care.

Iranians encourage domestic production of pharmaceuticals by levying the highest tariff on imports in the world, at almost 31% in addition to a 1.5% sales tax.
